DISABILITY INSURANCE

Short-Term Disability: Benefits are paid for up to 26 weeks
Long-Term Disability: Benefits begin if you are disabled for 26 weeks or longer

RETIREMENT PLAN - A 401(k) Savings Plan

Employee Contributions: Up to 50% of your compensation on a pre-tax basis
Employer Match: The Company will match 50% of your contribution up to 6% of your compensation
Employer Lump Sum Contribution: The Company may contribute a lump sum amount once a year as determined by the Board of Directors
